In this episode of the Less Insurance Dependence podcast, host Lester De Alwis welcomes Thomas Passalacqua, executive coach and founder of Ascend Professional Pathways. Thomas spent years as an award-winning sales consultant in the dental industry before leading business development and acquisitions at the Smile List, a growing dental group practice. Having seen dental practices from the inside out, both as a seller to them and as an acquirer, Thomas now helps practice leaders gain the clarity they need to grow and lead with confidence.

The conversation centers on a pattern Thomas saw repeatedly across both roles: practice owners rarely lack vision. What they lack is intentional execution against that vision. Clinicians are pulled into day-to-day patient care and reactive operations, leaving little energy or foresight for business growth. Thomas walks through the same pattern he observed evaluating practices for acquisition owners, staying in the moment rather than looking three to five years ahead, and rarely having full clarity on what they actually want for their practice, their partnerships, or their life.

Thomas introduces his ESSENCE Clarity Process, a framework for identifying a precise goal, assessing the obstacles standing in the way, pairing specific actions to each obstacle, and reflecting on what is and isn’t working as progress is made. Applied specifically to insurance independence, the process forces a practice owner to get granular: why do you actually want to get off insurance, what type of practice and patients do you want to attract, and what is the business plan to grow without that dependency? The episode closes with a direct challenge,e breaking free from dependence on insurance does not start with a fee schedule or a marketing plan. It starts with a leader willing to get clear.

Key Takeaways
  1. Most practice owners are not short on vision. They are short on execution against it. Clinicians are pulled into patient care and daily operations, leaving little energy for the practice’s business growth. The vision exists, but it just never gets broken down into tangible, actionable steps.
  2. The most common leadership mistake Thomas saw during acquisitions was being reactive rather than looking three to five years ahead. Practice owners who observe their practice as it currently is, rather than actively shaping where it could go, consistently end up with fewer options and less leverage when it matters most.
  3. The ESSENCE Clarity Process has four steps: identify the precise goal, assess the real obstacles, pair specific actions to each obstacle, and reflect on progress. Applied to insurance independence, it forces granular answers to why you want to get off insurance, what type of patients and practice you’re building toward, and what the actual plan is to get there.
  4. Team alignment is a direct downstream effect of leadership clarity. When a leader is clear on direction, every system, protocol, and interaction across the team starts complementing that vision instead of operating in siloed, disconnected ways. The entire team ends up rowing in the same direction.

  • 00:00:58 – From Selling to Practices to Helping Acquire Them: What It Revealed
    • Thomas’s experience spans both sides of the dental business relationship, first as a sales consultant to practices, then helping a dental group evaluate and acquire them. Across both roles, the same theme surfaced repeatedly: a lack of intentionality with the direction of the business.
    • Many clinicians hold a genuine vision for their practice but never translate it into tangible, executable steps for themselves or their team. The gap is not vision; it is execution and clear goal-setting toward that vision.

Thomas Passalacqua: In my experience working directly with practice owners at a variety of different stages, it’s really not being intentional with the direction of the business or the practice. Many clinicians have a vision, but they don’t necessarily act upon manifesting that vision. What are the actual tangible steps for them and their teams to take the practice from where they are to where they want it to go? Oftentimes, it’s just that lack of execution on that vision and a lack of clear goals to allow them to grow the way they wanted to grow when they started out.

  • 00:01:57 – Why Practice Owners Get Stuck: Wearing Too Many Hats
    • In the traditional owner-operator dental model, the lead doctor and team are wearing many hats simultaneously. Most of the day is spent reacting to patient care and daily operations, which is valuable, but it leaves little dedicated energy for business growth.
    • The missing piece in most practices is one or a few individuals, or a defined pocket of energy, specifically driving business growth alongside clinical growth. Without that, growth happens by accident rather than by design.

Thomas Passalacqua: In the traditional owner model, the lead doctor and the team are wearing many, many hats. So they’re mostly reacting day to day or focused on patient care, which is fantastic. But the energy behind business growth might be lacking, and there’s a lack of foresight or intention to grow in a particular way. There needs to be either one or a few individuals, or some energy, that is driving the business growth as well, alongside the clinical growth.

  • 00:03:11 – The Most Common Leadership Mistake Seen During Acquisitions
    • Evaluating practices for acquisition at the Smile List, the pattern Thomas saw most often was practice owners without a clear plan or goal, simply observing the practice as it currently existed rather than actively shaping its future.
    • Many owners stayed focused on the present moment without looking three to five years ahead, and often lacked full clarity on what they actually wanted for their business, their partnership structure, and their life. Thomas’s advice: take the time to explore all available options rather than being pressured into a decision.

Thomas Passalacqua: For acquisitions, most likely it was again not having that clear plan or goal, just kind of observing the practice as is, not having the intention to take it to where it could go. Many times,s practice owners would just stay in the moment and not look out three or five years. It’s also not having a clear vision of what you really want in your business, your practice, your life and knowing there are a lot of different options out there, especially when acquiring practices. I would say the intention of knowing precisely what you want for your practice, and then taking the time to explore those options, not being pressured to sell or forced into a decision, but trying to be the driver of where you want your practice to be.

  • 00:04:49 – Realistic Vision, Not Just Realistic Goals
    • Thomas extends the idea of realistic goal-setting to vision itself: a vision needs to be achievable and broken down into tangible, manageable steps and benchmarks that both the owner and the team can work toward together.
    • He encourages practice owners to set aside intentional time even just a couple of times a month,th to research business opportunities and stay informed, rather than remaining purely reactive and patient-centric to the point of losing sight of the business itself.

Thomas Passalacqua: A vision that is realistic to achieve, broken down into tangible steps and benchmarks that you and your team could work towards, that’s manageable. And educating yourself on what the options out there are, not staying so patient-centric that you’re losing sight of the business and the opportunities out there. I would encourage practice owners to take some intentional time to catch up on business opportunities. A couple of times a month, who are you talking to, where are you getting your information, how are you researching, nd being a little more proactive instead of reactive in the practice.

  • 00:05:42 – The ESSENCE Clarity Process: A Four-Step Framework
    • Thomas introduces the ESSENCE Clarity Process, developed from his own experience and from working with clients and business leaders navigating the same challenges. Step one: identify precisely the outcome you are looking to achieve, whether a long-term vision or an immediate, short-term goal.
    • Step two: assess the obstacles between where you are now and that goal. Step three: pair a specific action to each specific obstacle, since being action-oriented is what allows a leader to actually navigate past them. Step four: reflect on what is working and what isn’t as progress continues.
    • Applied to insurance independence specifically, the process forces a practice owner to get granular why do you want to get off insurance, what type of practice and patients are you building toward, how is your team engaging, and what is the actual business plan to grow without that dependency.

Thomas Passalacqua: The essence of it is identifying precisely the goal that you want to achieve, which could be a long-term vision or it could be a short-term, immediate goal. Step one is identifying precisely the outcome you’re looking to achieve. Then, from where you are at the moment, what are the obstacles in the way? Getting creative in assessing what’s actually preventing you from moving forward. Then it’s pairing actions to those obstacles, because being action-oriented allows you to navigate them. And as you’re making progress, it’s reflecting on what’s working and what’s not. Applied to practice leaders, it solidifies what they actually want to achieve, why they want to get off insurance, the type of practice and patients they want, and the business plan to grow without that insurance dependency.

  • 00:08:55 – How Leadership Clarity Changes the Way the Whole Team Sells the Practice’s Value
    • Once a leader is clear on direction, that clarity creates alignment across the entire team. Departments and individuals stop operating in siloed, disconnected ways because everyone understands the same vision and the reasoning behind it.
    • Every system, protocol, and patient interaction begins by complementing that shared vision rather than working against it. The result is a team that is collectively rowing in the same direction instead of pulling in conflicting directions.

Thomas Passalacqua: That alignment allows everyone to behave and perform in a way that’s complementary to that vision. There are no more siloed cultures or siloed behaviors in different departments once the clarity of where everyone is going and why is understood. Every action, every system, every protocol of that practice then complements that vision, and that team alignment is super important because it allows everyone to embody what it takes to collectively grow the practice in that direction. You’re not working against each other anymore; it’s all complementing the same vision and goal.

  • 00:10:20 – The One Move to Make This Week If Mindset Is the Real Bottleneck
    • Thomas’s advice for a dentist whose real obstacle is mindset rather than tactics: take specific, intentional time to outline exactly what you want to achieve for your practice and business over the next several years.
    • If the honest answer is "I don’t even have time to do that," Thomas’s point is direct: that is the bottleneck itself. Prioritizing the future and team alignment is the first step toward getting everyone moving in the same direction.

Thomas Passalacqua: With mindset work, it’s getting intentional. Take some specific time where you’re outlining what you actually want to achieve in the next number of years for your practice or your business. If you feel like, well, I don’t even have time to do that, then there’s the bottleneck right there. It’s being intentional, stepping out,t and prioritizing the future, prioritizing the direction, prioritizing everyone’s alignment for the team. As long as you stay ahead of that energy and intention, you’ll be able to start coordinating everyone going in that direction. Step one is trying to get a little clearer on exactly where you want to go and taking the time to do so.

  • 00:11:48 – How to Reach Thomas Passalacqua
    • LinkedIn: Thomas is highly active and encourages listeners to find him and send a message directly.
    • Website: ascendpropathways.com includes a message board and a contact page to reach Thomas by email.
    • Thomas’s closing note: amid the current focus on technology and AI, individuals remain at the center of any real progress. Staying sharp and intentional is what keeps a leader in control of the direction their practice takes.

Thomas Passalacqua: LinkedIn is a fantastic way to reach out to me I’m very active there, find me and send me a message. You can also visit my website, ascendpropathways.com. There’s a message board there, as well as my email through the contact page. Everyone is focused on the technology, the innovation, all the amazing new products and services coming out, which is fantastic. However, we as individuals are still at the center of all of that. And if we’re not sharp and intentional in trying to navigate that as best we can, we’re going to be easily overwhelmed and not in control of the direction we go. A lot of the work I do is on mindset and being intentional, so your actions and behaviors are actually taking you in the direction that’s meaningful for you.

Lester De Alwis: All the resources Thomas mentioned will be in the show notes, so you can grab them directly and get in contact with Thomas. Thomas, thank you so much for spending your time with us.

Thomas Passalacqua: Of course, Lester, it was great. Thanks so much for having me.

Patients really do not care about what the cost is if they receive a tremendous value — because the value you receive is more important than the money you spend. People don’t want to just buy something. What they want is a relationship.
